During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 07/21/2025. The on-site inspection began at 11:30 a.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 85 children present and 12 staff. The inspector reviewed eight children?s records and six staff records on-site.

This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/30/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment.

Staff B, who had been employed for eight months, did not have documentation that a central registry search has been requested. Staff C, who had been employed for more than five weeks, did not have documentation that a central registry search has been requested.

Plan of Correction: Staff B was hired prior to the new director. Original cannot be found. A new request was submitted for processing on 7/29/2025.

Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were seven children present in the Pre-K classroom, but five children were marked present on the attendance record.

Plan of Correction: The children were immediately properly documented on the PreK attendance sheet, and the teachers were reminded through a verbal warning that all children must always be properly signed into the classroom. ALL staff were re-trained on proper Attendance Recording.

Description: REPEAT VIOLATION
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

There is a hole in the wall of the school age classroom approximately 4 inches in size, leaving insulation and chipping drywall exposed.

Plan of Correction: An emergency work order has been placed to repair the hole.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials are required to be locked to prevent access by children.

One bottle of cleaning solution and one bottle of air freshener was observed on top of a paper towel holder in the bathroom of the two year old classroom.

Plan of Correction: Staff were retrained that all cleaning materials must always be locked away and inaccessible the changing tables must always stay locked. Management will ensure that classrooms are keeping hazardous substances are always locked away doing daily room checks.

Description: At least one staff in each classroom or area where children are present shall have current certification in CPR.

Staff A was alone with a group of three infants and was not certified in CPR.

Plan of Correction: The staff member and 3 children were moved and combined with Infant A classroom. The classrooms will remain combined until the staff member receives CPR training. A date for training has been scheduled for 8/6/25 @ 11am.

Description: At least one staff in each classroom or area where children are present shall have current certification in First Aid.

Staff A was alone with a group of three infants and was not certified in First Aid.

Plan of Correction: The staff member and the 3 children were moved and combined with Infant A classroom. The classrooms will remain combined until the staff member receives First Aid training. A date for training has been scheduled for 8/6/25 @ 11am.

Description: Shelter in place procedures shall be practiced a minimum of twice per year.

The most recent documented shelter in place drill was dated 05/23/2024.

Plan of Correction: A shelter in place drill was conducted on 7/25/2025.

Standard #: 8VAC20-780-550-F
Description: The center is required to practice lockdown procedures annually.

The most recent documented lockdown drill was dated 03/14/2024.

Plan of Correction: A lockdown drill was conducted on 7/24/2025. The center director will ensure that lockdown drills are conducted annually.
