During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on 07/24/2025 and completed on 07/24/2025. The on-site inspection began at 9:46am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and seven staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/01/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
On the preschool playground, two buckets holding cement to anchor poles were cracked and broken with sharp edges that could pinch skin. Two scooters were broken, leaving plastic sharp edges. Children were observed playing in the area at the time of inspection.Plan of Correction: Preschool playground cleaned up.
Standard #: 8VAC20-780-520-C Description: If diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.
Child #4 and child #5 did not have written parent authorization for their diaper ointment that was on site.Plan of Correction: Will fill out authorization each time for topical cream.
Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required information.
Two of five written injury reports did not include date/time the parents were notified. One of five reports did not include documentation of how the parent was notified.Plan of Correction: Will make sure date and time is filled out.
