During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 07/23/2025 and completed on 07/23/2025. The on-site inspection began at 9:00 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 3 staff. The inspector reviewed 3 children's records and 2 staff records on-site on 07/23/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/01/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
There is no documentation of a physical exam on file for child 2. Child 2 has been in care for more than 12 months.Plan of Correction: Center Lead Teacher will communicate with parents/guardians that the physical must be completed in order for child to return to school after 8/15/25.
Standard #: 8VAC20-780-60-A Description: Each center shall ensure that children's records contain all required elements.
The enrollment record for child 1 lacked the following required information:
*Work phone number for each parent;
*Name, address and telephone number of a second emergency contact;
*Name and phone number of child's physician;
*Home phone number for one parent.Plan of Correction: Center Lead Teacher will communicate with parents/guardians that all forms, including all fields on required forms, must be completed in order for child to return to school after 8/15/2025.
Standard #: 8VAC20-780-550-H Description: The center shall maintain a record of the dates of the practice drills for one year.
The emergency log for the facility lacked documentation of a practice fire drill for June 2025.Plan of Correction: June fire drill date has been added to the emergency log.
