During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated on July 9, 2025, and completed on July 30, 2025, in response to a complaint received by the licensing office on July 7, 2025, relating to allegations of staffing and supervision. The inspector reviewed 1 child record and 0 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 6, 2025. A POC submitted after this date will not appear on the public website.

Description: The required ratio for children ages 16 to 24 months is one staff per five children.

On July 7, 2025, in classroom #1, staff #2 and staff #3 were observed out of ratio (youngest child 16 months old) from approximately 8:53am until 11:53am with:
-11 children from approximately 8:53am until 9:13am.
-12 children from approximately 9:13am until 11:53am.
Witness #1, witness #2 and witness #3 confirmed that classroom #1 was out of ratio from 8:53am until 11:53am. The center's face to name transition sheet confirmed that classroom #1 was out of ratio from approximately 8:53am until 11:53am.

Plan of Correction: All staff will be trained to not accept a child if their class is at max ratio capacity.

Description: The required ratio for children ages 3 years old up to school age is one staff per ten children.

On July 7, 2025, in classroom #2, staff #1 and staff #4 were out of ratio (youngest child 3 years old) from approximately 9:24am until 12:08pm with:
-22 children from approximately 9:24am until 9:50am.
-24 children from approximately 9:50am until 12:08pm.
Witness #3 and witness #4 observed classroom #2 out of ratio from approximately 9:24am until 12:08pm. The center's face to name transition sheet confirmed that classroom #2 was out of ratio from approximately 9:24am until 12:08pm

On July 7, 2025, in classroom #3, staff #1 was out of ratio (youngest child 3 years old) from approximately 3:00pm until 4:27pm with:
-15 children from approximately 3:00pm until 4:06pm.
-14 children from approximately 4:06pm until 4:10pm.
-13 children from approximately 4:10pm until 4:16pm.
-12 children from approximately 4:16pm until 4:25pm.
-11 children from approximately 4:25pm until 4:27pm.
Witness #4 observed classroom #3 out of ratio from approximately 3:00pm until 4:27pm. The center's face to name transition sheet confirmed that classroom #3 was out of ratio from approximately 3:00pm until 4:27pm.

Plan of Correction: All staff will be trained to not accept a child if their class is at max ratio capacity.
