An unannounced inspection was completed on 07/08/2025 in response to a complaint received by the licensing office on 06/30/2025 relating to allegations of staffing and supervision. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and procedures for emergencies and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/08/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-C Complaint related: No Description: A tuberculosis (TB) screening is required every two years from the last screening. The updated TB screening for S1 was 19 months late. Plan of Correction: All staff now have TB on file.
Standard #: 8VAC20-780-240-B Complaint related: No Description: Staff shall complete orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities. The personnel record for S2 employed for 2 months did not contain documentation of orientation training. Plan of Correction: All staff now have documentation of orientation on file.
Standard #: 8VAC20-780-550-P Complaint related: No Description: The center shall maintain written records of children?s injuries that contains all of the
required information. Injury reports for C1 dated 05/25/2025 and 06/27/2025 did not document actions to prevent recurrence and the time of parental notification.Plan of Correction: I have been checking all reports for accuracy before signing.
