Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Director is scheduled to attend CDC Phase 2 training with OCCHS in August. Recommended AviQ training when renewing MAT certifications and to keep all of a child's medications and action plan together.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 8/5/2025, as a part of the licensure period. The on-site inspection began at 12:11pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were 85 children present and 20 staff. The inspector reviewed 8 children?s records and 4 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-260-B
Description:	The center shall have annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply and sewage disposal system. Most recent environmental sanitation report is dated 7/2024.
Plan of Correction:	Director contacted health department to schedule environmental sanitation inspection. Director will track due dates of inspections, and schedule annually.
Standard #:	8VAC20-780-510-L
Description:	REPEAT VIOLATION Medication shall be kept in a locked place using a safe locking method that prevents access by children. In classroom #1, 2 medications were stored not locked.
Plan of Correction:	Staff relocated medications to designated locked storage, during the inspection. Director will review medication storage policy with all staff and periodically check classrooms for storage compliance.
