During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 8-4-25. The on-site inspection began at 11:10am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 89 children present and 14 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/13/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-M
Description: (Repeat Violation) The Center is required to maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that is accessible to all staff in each group or area where children are present.

The allergy list did not contain two of Child #1`s diagnosed food allergies.

Plan of Correction: The allergy list has been
updated to reflect the two
additional allergy items.

Standard #: 8VAC20-780-280-B
Description: (Repeat Violation) The Center is required to keep hazardous substances in a locked place using a safe locking method that prevents access by children.

During inspection in the unlocked kitchen directly off the front hallway where children have access, 2 containers of detergent, 1 container of multi-purpose cleaner, 2 containers of disinfectant, and 1 container of sanitizer were stored in the open area under the sink in an unlocked area.

retrained in safety protocols
to ensure kitchen door locked
at all times

Standard #: 8VAC20-780-340-F
Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Upon entering the Infant B classroom, 2 children (ages 10 months old and 13 months old) were observed sleeping in cribs and no staff were present on the Infant B side of the half wall in the classroom.

Plan of Correction: 2 staff memebers were in the
classroom with 8 children present; our
Licensing guidelines up until now have
been as long as the room is not
fully seperated that the room
was one large classroom. The room is
not separated by a full partition, there
is an open walkway space in between
both sides of the room creating one
large classroom. Going forward, we
will treat the classroom as two
separate rooms and operate 1:4 ratio
on both sides of the room at all times.

Standard #: 8VAC20-780-350-B-1
Description: The Center is required to maintain the required supervision ratio of 1 staff to 4 infants age birth to 16 months old whenever children are in care.

At the time of inspection, in the Infant A classroom, 6 children (under age 16 months old) were observed with 1 staff member on the Infant A side of the classroom which is divided by a half wall.
