During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on 7/23/2025 and completed on 8/6/2025 in response to a complaint received by the licensing office on 7/18/2025 relating to allegations of forbidden actions. The inspector reviewed 1 child record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and physical plant, and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 13, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-50-B Complaint related: No Description: Records and reports on staff shall be maintained and made accessible for two years after separation from employment. Administration reported Staff #2 left the center two weeks prior to this inspection. The record for Staff #2 was not accessible during the inspection. Plan of Correction: Moving forward, we have implemented a centralized storage system to ensure all separated staff records are properly archived and easily accessible for the required two-year period.
Standard #: 8VAC20-780-270-A Complaint related: No Description: The center is required to ensure equipment inside the center is maintained in a clean, safe, and operable condition. Soft blocks located in the Steam lab used by multiple classrooms, were observed to be torn at the corners. The break in the material creates a potential choking hazard. Plan of Correction: The blocks were removed from the Steam lab during inspection. We will conduct a comprehensive inspection of all materials and toys in the STEAM lab and other shared classroom areas to identify and remove any additional damaged or worn items that pose safety risks.
