An unannounced inspection was initiated on 7/31/2025 and completed on 8/11/2025 with a local agency in response to a complaint received by the licensing office on 7/28/2025 relating to allegations of supervision and forbidden actions. The inspector reviewed one staff record on-site on 7/31/2025. This inspection included document review, tour of the facility and interviews.
The preponderance of evidence gathered during the investigation supports one of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/18/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff member #1 was observed sleeping in the classroom at least two different dates in two different classrooms(Toddler Classroom and Two Year Old Classroom). Staff #1 was alone with the children during naptime and was responsible for ensuring their care, protection and guidance.Plan of Correction: o Staff #2 responded within 10?15 seconds, entering the classroom immediately.
o Staff #3 remained in the office to monitor the classroom via camera during Staff #2?s response.
o Staff #1 was promptly removed from the classroom and sent home.
o Following further review and Staff #1's admission to being asleep for an extended period, Staff #1 was terminated.
o All staff have been re-trained on supervision expectations, including protocols for rest periods and alertness requirements.
o Leadership has implemented enhanced daily walkthroughs and monitoring proce-dures to ensure compliance with supervision regulations.
o Leadership will continue to use video surveillance and walk throughs during rest periods to verify active supervision is maintained at all times.
Standard #: 8VAC20-780-350-G Complaint related: Yes Description: Staff shall be counted in the required staff-to-children ratios only when they are directly supervising children.
While staff #1 was the only staff member present in the Infant/Toddler classroom, staff #2 observed staff #1 nodding off during naptime. Staff #1 confirmed this allegation and admitted to being asleep for approximately 10-15 minutes sometime in late May, early June 2025. Two additional staff stated they had observed Staff #1 sleeping in the infant/toddler classroom and had reported it to management.
On June 13, 2025, two staff members (Staff #2 and Staff #3) observed Staff #1 on video sleeping for a few seconds during the children's naptime in Preschool Explorers B classroom (2 year old classroom with 3 children present).. Staff #1 was the only staff member present in the classroom and was not directly supervising children while sleeping.. One of the staff members had to wake her up. One staff member stated "We saw her head down, she dozed off." Staff #1 admitted she was asleep in the 2 year old classroom that day and stated she was asleep for approximately 10-15 minutesPlan of Correction: o Staff #2 instructed Staff #1 to stand up, walk around, and engage in active supervi-sion duties.
o Staff #2 provided clear verbal correction and guidance to Staff #1 on the im-portance of alertness and maintaining supervision, especially during rest periods.
o Staff #1 was immediately removed from the classroom and directed to take a lunch break due to fatigue.
o Staff #2 increased monitoring of all classrooms to ensure staff were alert, en-gaged, and always maintaining appropriate supervision.
