Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Standards 510.a though P were discussed with the assistant director to include medication authorizations, over-the-counter medications and expiring medications and/or parental authorizations. Standard 280.B regarding storage of potentially hazardous substances in the front office area was discussed. Items stored in the first aid kit that cannot be used/applied to children without written parent authorization was discussed to ensure topical ointments are not inadvertently applied to children's injuries.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 08/05/2025, as a part of the licensure period. The on-site inspection began at 11:45 AM and ended at 2:55 PM. The inspector reviewed compliance in the areas listed above. There were 58 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: ? document review, ? tour of the facility, ? interviews, and ? observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on August13, 2025. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-60-A
Description:	Repeat violation Each center shall maintain and keep at the center a separate record for each child enrolled that contains the required information. The record provided for child 2 did not contain the street address for one person to be contacted if a parent cannot be reached. The record for child 3 did not contain the parent agreement stating that the parent will inform the center within 24 hours or the next business day of after his child or any member of the household has developed a reportable communicable disease.
Plan of Correction:	Provider response Center Director and Asst. Director will ensure that all files are updated and corrected. Center Director will ensure the correct verbiage is on the forms that are filled out before enrollment.
Standard #:	8VAC20-780-70
Description:	Staff records shall be kept for each staff person that includes the date of separation from employment. The record provided for staff 1 did not include the date of separation from employment.

Standard #:	8VAC20-780-430-I
Description:	If combs, toothbrushes, or other personal articles are used, they shall be individually assigned. There was a pacifier in the young toddlers classroom that was not individually assigned.
Plan of Correction:	Provider response Center Director will review guidelines with the teachers and check the classrooms to ensure all cups pacifiers are labeled.

Description:	The center's procedures for administering medication shall include methods to prevent the use of outdated medication. The center's procedures for administering medication did not include their methods to prevent the use of outdated medication. There was emergency asthma medication for child 6 that expired in June 2025.
Plan of Correction:	Provider response A standard operating procedure will be written to accompany the Monthly Medication Authorization checklist to include the procedure to discard expired medication
Standard #:	8VAC20-780-560-F
Description:	When centers choose to provide snacks, they shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the USDA. Children in the young toddler classroom were given only graham crackers and water for afternoon snack - 1 food group component.
