An unannounced inspection was initiated on 7/23/2025 and completed 8/7/2025 in response to a complaint received by the licensing office on 7/21/2025 relating to allegations of supervision and the care and protection of children. The inspector reviewed 1 child record and 2 staff records on-site. The inspection included document review, interviews, observations, and a tour of the program.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 8/14/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Complaint related: No Description: REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for reach child enrolled that contains all required information.
Child #1, enrolled for 2 years, did not have all required information for 2 emergency contacts.Plan of Correction: We kept the information on the file.
Standard #: 8VAC20-780-70 Complaint related: No Description: Staff records shall contain documentation that two or more references were checked before employment.
At the time of the inspection, the record for Staff #1 and Staff #2, employed for approximately 30 days, did not contain two reference checks.Plan of Correction: Already in the file.
Standard #: 8VAC20-780-340-D Complaint related: Yes Description: In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present.
On 7/15/2025, Staff #1 and Staff #2 were overseeing the Pre-K classroom for approximately 2-3 hours after the lead staff had left. Based on staff interviews, lead qualifed staff work in the morning and assistants work in the evening. Staff #1 and #2 did not have documentation of education or required training hours to verify if they were lead qualified.Plan of Correction: Corrected. We got Staff #1 and #2 educational certifications and keep on the file.
Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On 7/15/2025, Child #1 (5 yrs of age) was alone in the hallway, approximately 1-2 feet from the classroom door, for less than 5 minutes.
Based on multiple interviews, Staff #1 was approximately 10 feet inside the classroom and unaware the child was in the hallway. Staff #2 had been with the child in the hallway and left to help another staff member in a different area. The child was found by the Center Director.Plan of Correction: Talked to all the staff-teachers about child safety. Reviewed the center policy.
