During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 7/28/25 and completed on 7/28/25. The on-site inspection began at 9:10 a.m. and ended at 10:40 a.m. The inspector reviewed compliance in the areas listed above. There were 60 children present and 13 staff. The inspector reviewed five children?s records and five staff records onsite. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/15/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-80-A
Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:

1. In the twaddlers class, ages two-years-old, there were five children present, the written record reflected there were four.

2. In the Prepper class, ages two and three-years-old, there were six children present, the written record reflected there were five.

Description: Repeat Violation

Areas and equipment of the center, inside and outside shall be maintained in a clean, safe, and operable condition.

Evidence: On the younger playground a section of the artificial grass rug was sticking out from the mulch, posing a trip hazard.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation

Where playground equipment is provided, at least six inches of resilient surfacing shall be under equipment with moving parts or climbing apparatus to create fall zone.

Evidence: On the older playground there was less than one inch of mulch in front and on all sides of the slide and fall zones of the composite play structure. The mulch has deteriorated into compacted dirt and is no longer resilient. Six inches of resilient surfacing is required in the fall zones of the composite play structure including slide exits and stairs.
