During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated on 7/29/25 and completed on 7/29/25. The on-site inspection began at 8:30 a.m. and ended at 10:30 a.m. The inspector reviewed compliance in the areas listed above. There were 47 children present and 10 staff. The inspector reviewed five children?s records and five staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/18/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-M Description: Repeat Violation
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present.
Evidence: The allergy list only identified one food allergy for child #1, according to documentation the child had three additional diagnosed food allergies that were not on the list.Plan of Correction: The site director will ensure a current list of all known allergies, sensitivities, and dietary restrictions is maintained and posted in areas where children are present in the program.
Standard #: 8VAC20-780-60-A-8 Description: Each center shall maintain and keep a separate record for each child enrolled which shall contain a written care plan for each child with diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record for Child #5 contained documentation of a diagnosed food allergy to eggs. There was no written allergy care plan from a physician on record. Child #5 had been enrolled in the program for two months.Plan of Correction: The site director will ensure a written care plan is maintained for any child diagnosed with food allergies with instructions provided by a physician.
Standard #: 8VAC20-780-80-A Description: Repeat Violation
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: There were 41 children present, the written record reflected there were 39.Plan of Correction: Staff received training on attendance, emphasizing that a written record must be maintained at the arrival and departure of each student. All staff were trained on attendance procedures and policy.
