An unannounced inspection was initiated on 6/6/25 and completed on 8/8/25 with local agencies, in response to a complaint received by the licensing office on 5/30/25 relating to allegations of forbidden actions. This inspection included: document review, tour of the facility, and interviews,
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
