During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical assistance was provided and standards reviewed in the following areas: 8VAC20-780-270. Building maintenance and 8VAC20-780-500. Hand washing and toileting procedures.
- Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 8/5/25. The on-site inspection began at 11:05am and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were 89 children present and 17 staff. The inspector reviewed eight children?s records and six staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/26/25. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1 Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for Staff #1 (employed for two months) was dated seven weeks after their first day of employment.Plan of Correction: Employee had already re-signed sworn disclosure prior to inspection. Moving forward, the management team will have a second member of management verify staff files prior to their first day of employment to ensure all paperwork is complete.
Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
The record for Child #1 (enrolled for 11 months) did not contain documentation of additional immunizations, which is required.Plan of Correction: Updated vaccinations were provided the day of the visit. Moving forward, the management team will suspend care when a family does not provide necessary immunization updates.
Standard #: 8VAC20-780-160-A Description: Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening.
The TB screening for Staff #1 (employed for two months) was not submitted at the time of employment, which is required.Plan of Correction: Staff member was sent home on first day while doing new hire paperwork and did not return until after he received his TB screening results. Moving forward, the management team will have a second member of management verify staff files prior to their first day of employment to ensure all paperwork is complete.
Standard #: 8VAC20-780-70 Description: The required information shall be kept in each staff?s record.
One of the two required references for Staff #1 (employed for two months) did not contain a date of contact, which is required.Plan of Correction: This was corrected during the visit.
