During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
The Licensing Inspector discussed background checks.

Comments:
An unannounced renewal inspection was conducted on 8/12/2025. The insepctor reviewed 5 child records, 6 staff records, and 6 board member records on-site. The inspection included document review, interviews, observations, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 8/19/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The initial immunization record for Child #2 was dated almost 4 months after the first date of attendance.

Plan of Correction: We kept

Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every 6 months for children under the age of 2 years.

An updated immunization record was not obtained for Child #4 until 46 days after it was due.

Plan of Correction: We are working on that.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: work phone number and place of employment.

The record for Child #1, enrolled for 3 weeks, did not contain employment information for one parent.

Plan of Correction: We kept in the folder all information

Standard #: 8VAC20-780-370-5
Description: Play spaces for infants may include infant seats and exercise chairs or saucers, but not walkers.

In the Infant classroom, a child was observed in a walker with wheels.

Plan of Correction: We took it out the walker with wheels.

Standard #: 8VAC20-780-420-E-3
Description: Staff shall request at least annually parent confirmation that the required information in a child's record is up to date.

Child #2, enrolled for almost 2 years, and Child #3, enrolled for more than 2 years, did not have documentation of an annual update.

Plan of Correction: We are working on that. Getting sign from parents. We will be done in two days.
