Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site focused inspection was initiated on August 13, 2025 and completed on August 13, 2025, as a part of the current licensure period. The on-site inspection began at 9:00am and ended at 10:25am. The inspector reviewed compliance in the areas listed above. There were 32 children present and 15 staff. The inspector reviewed 3 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 20, 2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-A
Description:	The center is required to have employees undergo background checks every five years. The most recent criminal background check for staff #2 was overdue by two years. There is no central registry documentation available for review for same staff member.
Plan of Correction:	~Conducted a review of all staff members files and ensure that their background checks are not more than five years old. All staff members with background checks older than five years will renew their background checks immediately. ~Staff #2 initiated local, central registry and FBI background check renewal on 8/14/25.
Standard #:	8VAC20-780-160-A
Description:	REPEAT VIOLATION SYSTEMIC DEFICIENCY Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The TB test for Staff 1 was completed two months after hire.
Plan of Correction:	Noted that all staff members must have negative Tb test with 30 days of employment, but before coming into contact with children. Central Office has corrected this issue by ensuring that all newly hired staff members have TB testing completed prior to staff member's start date(s). No correction can be made for Staff 1.
Standard #:	8VAC20-780-160-C
Description:	A tuberculosis (TB) screening is required every two years from the last screening. Staff 2's updated screening was due 2 years ago, Staff 3's updated screening was due 13 months ago. The updated screenings have not yet been completed.
Plan of Correction:	All staff members and volunteers will be required to get TB testing regardless of the position here at Hopkins House. A review of staff member files that to ensure current negative TB test results on on file. Special focus on those hired within the past two to three years.
Standard #:	8VAC20-780-340-A
Description:	REPEAT VIOLATION SYSTEMIC DEFICIENCY When staff are supervising children, they shall always ensure their care, protection, and guidance. In the infant classroom, the egress door was blocked by three cribs. During the time of the inspection, there were no children asleep in the cribs.
Plan of Correction:	~On the spot correction conducted to ensure the egress route is not blocked. ~Collectively inform the staff members of the importance of maintaining clear ingress and egress routes for emergency purposes. ~Follow-up daily to ensure the routes remain cleared.
