An unannounced monitoring inspection was initiated on 8/11/2025 and completed on 8/15/2025, in response to a self-report received by the licensing office on 8/6/25 relating to an allegation of roughly handling children, and behavioral guidance. The inspector reviewed 3 staff records on-site. This inspection included;document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background searches and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/22/25. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-A Description: The center is required to have employees undergo a background check every five years.
The most recent central registry check, for an out of state residence, for Staff A was overdue by 28 months.Plan of Correction: The staff member involved in the self-reported incident was immediately placed on administrative leave and subsequently terminated.
The Regional Manager will conduct an audit on all staff files to ensure all files are
in order. During the Regional Manager?s monthly center visits, new staff files will be reviewed to verify they too are in compliance.
Center Director, in conjunction with the Regional Manager, will utilize a tracking system to ensure all background checks are submitted in a timely manner.
Standard #: 8VAC20-780-400-B Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Multiple staff confirmed that Staff A "yelled" or used a loud/harsh tone when telling a child "You can't say no to me" and "I don't like the way you talked to me".Plan of Correction: The staff member involved in the self-reported incident was immediately placed on administrative leave and subsequently terminated.
All staff will be retrained on Positive Guidance and Interactions with children
policy. The training will include real examples of challenging behaviors and appropriate ways to address.
Center administration will conduct random and ongoing observations in each classroom to verify policies are being implemented properly.
