During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 8/12/2025. The on-site inspection began at 7:53am and ended at 9:30am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/26/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-140-A
Description: Each child shall have a physical examination by or under the direction of a physician: 1. Before the child's attendance; or 2. Within 30 days after the first day of attendance.

Child 2 has been in care for approximately 3.5 months, and did not have documentation of a health evaluation.

Standard #: 8VAC20-780-160-A
Description: Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Staff 2 was hired in May of 2025, but their TB was dated in December of 2023.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Staff 2 was standing in the hallway outside of Room One, while the teacher left the classroom. Staff 2 was standing in the hallway listening to the conversation between the licensing inspector and the administrator instead of actively supervising the classroom.

Description: Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Observed two children putting toys away and going straight to their tables for breakfast, before washing their hands. Also observed a child that just arrived to care went to the table for breakfast before washing their hands.
