During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated on 8/19/2025 and completed on 8/20/2025. The on-site inspection began at 9:39 am and ended at 2:50 pm. The inspector reviewed compliance in the areas listed above. There were 98 children present and 17 staff. The inspector reviewed 9 children?s records and 9 staff records on-site, and 4 officer records electronically on 8/20/2025. This inspection included document review, a tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/27/2025.
A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There were no physical examinations in the records of child #1, who had been enrolled for three months, child #2 who had been enrolled for over one month, child #7 who had been enrolled for over one month, and child #8 who had been enrolled for three months.Plan of Correction: having all children have an updated physical exam and enrollment paperwork
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening.
Staff #8's updated screening was due two months ago. The updated screening had not yet been completed.Plan of Correction: Staff member received TB screening
Standard #: 8VAC20-780-70 Description: Records shall be kept for each staff member that contain the required information.
Staff #3, who has been employed for over 2 months, had one reference that was completed three days after employment and one reference that was completed 15 days after employment. Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #2 and Staff #3, employed for over two months, did not have documentation to demonstrate that the staff completed the required orientation. Administration stated the orientation was completed but not signed by the staff.Plan of Correction: moving forward making sure all staff references and documentation is done on time and before starting the job
Standard #: 8VAC20-780-350-G Description: Staff shall be counted in the required staff-to-children ratios only when they are directly supervising children.
On 8/19/2025, Staff #8 and Staff #10 were supervising 12 children in the toddler classroom during the designated rest period. Staff #10 fell asleep leaving Staff #8 to directly supervise the children for a period of less than five minutes.Plan of Correction: Making sure that all staff understand direct supervise
Standard #: 8VAC20-780-520-C Description: If diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.
