An unannounced monitoring inspection was initiated and completed on 8/19/2025 in response to a self-report received by the licensing office on 8/8/2025 relating to forbidden actions. The inspector reviewed 3 children?s records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: The center is required to have employees undergo a background check every five years.
The fingerprint background checks for staff #1 was completed 5 months, after due date.Plan of Correction: Employee fingerprint background check completed in 6/2025 when it was noticed during staff file audit. Staff files will routinely be audited and corrections documented.
