During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussed with the center updating their parent/staff handbooks, route to the playground, FARE allergy action plans, and supervising vs. non-supervising staff sleeping in the classrooms during nap time.

Comments:
An unannounced, on-site monitoring inspection was initiated on 08/21/2025 and completed on 08/21/2025. The on-site inspection began at 12:30 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 11 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/02/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.011-F
Description: The inspection summary and violation notice shall be posted in a conspicuous place on the licensed premises.

The inspection summary and violation notice from 07/28/2025 was not posted on the premises.

Plan of Correction: The center was awaiting a clean copy of the notice to be sent via email. Copy was received on 8/21 and posted.

Standard #: 8VAC20-780-350-B-1
Description: The ratio for children aged birth up to 16 months is 1:4 (staff:children).

Classroom A had a ratio of 2:9 (staff:children). They were 1 child over the allowed ratio for that age group.
Classroom B had a ratio of 2:10 (staff:children). They were 2 children over the allowed ratio for that age group.

Plan of Correction: The center has plenty of infant staff on site this day. Staff had stepped out to warm bottle. Staff has been reminded to secure a backup staff before leaving classroom.
