An unannounced, on-site monitoring inspection was initiated and completed on August 25, 2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Staffing/Supervision and Background Checks. The on-site inspection began at 2:00 pm and ended at 3:28 pm. The inspector reviewed compliance in the areas listed above. There were twenty-five children present and five staff. The inspector reviewed eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 3, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record for Staff #1 had documentation of requesting a out of state background check three weeks after beginning employment. The record for Staff #5 had documentation of requesting a out of state background check one month after beginning employment. The record for Staff #6 had documentation of requesting a out of state background check three months after beginning employment. The record for Staff #7 did not have documentation of requesting a out of state criminal history check and the individual has been working for more than a month.
