Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was completed on 08/27/2025, as a part of the licensure period. The on-site inspection began at 10:10 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and 18 staff. The inspector reviewed 7 children?s records and 6 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/03/2025. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-140-A
Description:	Each child shall have a physical examination under the direction of a physician within 30 days after the first day of attendance. The file for child #1, who had been attending for 79 days, did not have documentation of a physical examination.

Standard #:	8VAC20-780-280-B
Description:	Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. In the toddler class, various cleaning materials were found to be unlocked on a shelf in the bathroom. Additional cleaning supplies were found to be unlocked in the First Steps classroom on a shelf above a table, and in the Pre-K classroom on a shelf in the bathroom.

Description:	Staff shall request at least annually, parent confirmation that the required information in the child's record is up to date. The records for child #2, #4, #6, and #7 did not have an annual record review to confirm each child's record was up-to-date, as required by standard 8VAC20-780-60 A-11.

Description:	When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. In the Preschool 2 classroom, one lunch box was found without a date on it, and another lunch box was found without a name or date on it.
