An unannounced inspection was initiated on 08/26/2025 and completed on 08/28/2025 in response to a complaint received by the licensing office on 08/25/2025 relating to allegations of Special Care Provisions. This inspection included document review, a tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
