During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site focused inspection was initiated and completed on 08/19/2025, as a part of the licensure period. The Department issued a Special Order on 07/17/2025 to place the licensee on probation for a period of six months as a result of high-risk violations. The on-site inspection began at 11:25am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-550-P Description: Centers are required to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all required information. 6 of 10 injury reports reviewed did not contain documentation of the date and time when parents were notified. 1 injury report did not contain staff and parent signatures or two staff signatures- the report only contained one staff signature. Plan of Correction: Staff will be retrained on how to complete injury reports during staff meeting tomorrow.
