During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on August 25, 2025. The on-site inspection began at 10:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included a tour of the facility, interviews, observations, and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/5/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years.

The most recent sworn statement for staff #4 was overdue by a year and three months. The most recent sworn statement for staff # 5 was overdue by four months.

Standard #: 8VAC20-780-160-A
Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2 TB was dated 10 months prior to the date of employment. Staff #2 has been employed for four months.

Description: A tuberculosis (TB) screening is required every two years from the last screening.

The record for staff #4 did not contain an updated tuberculosis (TB) screening. Staff #4 updated screening was due 30 days ago.

Standard #: 8VAC20-780-270-A
Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1. Rest mats stored in the preschool classroom, were torn exposing the inside cushion material.
2. Multiple areas of the wooden deck outside of the Two?s and Three?s building, had protruding nails and splintered wood.
3. A wooden climbing structure located on the school age playground, had a floor plank that was lifted from the base that could be a potential tripping hazard.
