During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on September 2, 2025. The on-site inspection began at 10:05 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were forty-six children present and eight staff. The inspector reviewed five children?s records and five staff records on-site This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 11, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for more than a year, was missing a parent's work phone number. The record for Child #4, enrolled for more than a year, was missing documentation of verifying proof of child's identity. Plan of Correction: The missing work phone number belonged to a parent who is unemployed and does not have a work number. The director wrote on the registration form "unemployed" to reflect parent record is in compliance.
All families are required to show proof of identity prior to enrollment and the director will ensure this information is documented on the registration form that each parent completes prior to the child's start date. File was corrected on following day with the parent.
Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. A prescription medication was on site for a child and the authorization had expired two months ago. The authorization had not been renewed and the medication was not returned or disposed of as required. Plan of Correction: The child had been out of care for the entire summer and the child's first day back in care was on the first day of school. Moving forward the director will send home all medication prior to a child taking an extended leave of absence to ensure authorizations do not expire while not in care. The director ensured the parent took the medication home upon pick up on the first day of school and provided the parent with a new medication authorization to complete and return if the parent chooses to bring the medication back.
