During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced on-site inspection was initiated and completed on 9/3/25. The on-site inspection began at 11:00 and ended at 12:30. The inspector reviewed compliance in the areas listed above. There were 27 children and 6 staff present. The inspector reviewed 6 children?s records. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-I
Description: The center administers only those drugs that were dispensed from a pharmacy and maintained in the original , labeled container.

Child # 6 had a prescription medication at the center and it was not in the original, labeled container.

Plan of Correction: Staff called the parent to bring in the original, labeled container during the inspection.

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center.

There was a medication for a child and the child was no loner enrolled and the authorization was expired.

Plan of Correction: Staff removed the medication from the locked cabinet and will be disposing of it.
