During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on September 4, 2025, and completed on September 4, 2025, as a part of the licensure period. The on-site inspection began at 1:00 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and six staff. The inspector reviewed five children?s records and six staff records on site. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years. The most recent national criminal fingerprint background check for staff #6 was overdue by four days. There was no documentation that the national criminal fingerprint background check had been requested.

Plan of Correction: Per the director: I will set up an account on the portal and submit all background request to keep documentation.

Standard #: 22.1-289.035-B-3
Description: Repeat
The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for three weeks, did not have documentation the central registry was requested prior to employment. The results of the central registry were received 08/21/2025.

Plan of Correction: Per the director: I will set up an account on the portal and submit all background request to keep documentation.

Standard #: 8VAC20-780-160-C
Description: Repeat
At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening. An updated tuberculosis screening for staff #2 was due four months ago. The updated tuberculosis screening has not yet been completed.

Plan of Correction: Per the director: I will have the TB renewed.

Standard #: 8VAC20-780-245-A
Description: Repeat
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #2 obtained 10 of the required 16 hours of training between the period of September 1, 2024, through August 31, 2025.

Plan of Correction: Per the director: I will complete the missing hours of training.

Standard #: 8VAC20-780-260-B
Description: After the first license, annual approval from the health department shall be provided. An annual report of Environmental sanitation was due five months ago. The most recent environmental sanitation report was dated 03/27/2024.

Plan of Correction: Per the director: I will call the health department and request they complete the inspection.

Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. An area of the playground had multiple seams of the old artificial turf mat coving that were curled upward creating a tripping hazard.

Plan of Correction: Per the director: We will block off the area until the new artificial turf and be installed in that area. Children will not play in that area.
