During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site focused inspection was initiated on September 4, 2025, and completed on September 4, 2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to supervision, operational responsibilities, medication, and procedures for emergencies. The on-site inspection began at 3:15 pm and ended at 4:25 pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and six staff. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-F
Description: The medication authorization shall be available to staff during the entire time it is effective. A prescription inhaler was observed for child #1 and no written parental authorization was present for the medication.

Plan of Correction: Per administration, The second page to the authorization may not have been printed and may be at the main office. If not we will obtain the parents authorization to be kept on site.
