Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 9/5/2025, as a part of the licensure period. The on-site inspection began at 2:27pm and ended at 4:55pm. The inspector reviewed compliance in the areas listed above. There were 120 children present and 25 staff. The inspector reviewed 10 children?s records and 6 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-4
Description:	REPEAT VIOLATION The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #3, employed for more than 30 days, did not have documentation of requesting a central registry check or criminal history check from a state, resided in past 5 years.
Plan of Correction:	Employee received out of state background clearances and they are now on file. A new checklist has been created to use going forward for all files during hiring process with two person check for complete.
Standard #:	8VAC20-780-160-A
Description:	Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening and shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. TB screening document for staff #3 did not identify the person and was not signed by a physician, physician's designee, or an official of the local health department. TB screening for staff #6 was obtained 120 prior to the time of employment.
Plan of Correction:	Employee #3 has submitted a new print out of clearance with name and expecting additional doctors note with date and signature. Employee #6 will have new TB clearance completed with missing information. A new checklist has been created to use going forward for all files during the hiring process with two person check for completion.
Standard #:	8VAC20-780-60-A
Description:	Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Child #8' record did not contain information for a 2nd emergency contact person.
Plan of Correction:	Child #8s parents have supplied required information. Two employees will review all new student files to verify they are completed before student starts, using a checklist.
Standard #:	8VAC20-780-70
Description:	REPEAT VIOLATION Staff records shall contain name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center. Staff #3's record did not have information of an emergency contact.
Plan of Correction:	Employee #3 has completed emergency contact information. A new checklist has been created to use going forward for all files, during the hiring process with two person check for completion.
Standard #:	8VAC20-780-90-A
Description:	A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Child #8s record did not have parental signature of notification of communicable disease.
Plan of Correction:	Parent has signed agreement. To employees will review all new student files to verify they are completed before student starts, using a checklist.
