Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Technical Assistance:

~ Potting soil with additives should be locked, inaccessible to children. ~ Typed inspection summary should be posted.

Comments:

An unannounced, on-site monitoring inspection was initiated on September 8, 2025 and completed on September 8, 2025, as a part of the current licensure period. The on-site inspection began at 2:45pm and ended at 4:15pm. The inspector reviewed compliance in the areas listed above. There were 50 children present and 5 staff. The inspector reviewed 5 child files and 5 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-60-A
Description:	Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of Child A had not been updated in over a year.
Plan of Correction:	We will get an update at pickup.
Standard #:	8VAC20-780-70
Description:	Staff files will contain the name, address, and telephone number of a person to be notified in an emergency. Staff #2 did not have the name, address and phone number of a person to be notified in an emergency.
Plan of Correction:	We have gotten an emergency contact for the file.
Standard #:	8VAC20-780-245-L
Description:	There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Staff 3, who is the only staff member to have DHO, had documentation of the training taken over 3 years prior.
