Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed requirements of annual parent confirmation of required information in the child's record is up to date.

Comments:

An unannounced, on-site monitoring inspection was initiated on 09/09/2025 and completed on 09/09/2025, as a part of the licensure period. The on-site inspection began at 11:20am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 113 children present and 24 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-160-C
Description:	A tuberculosis (TB) screening is required every two years from the last screening. Staff #2's updated screening was due eight days ago. The updated screening has not yet been completed.
Plan of Correction:	Staff member will obtain an updated TB screening.
Standard #:	8VAC20-780-70
Description:	Documentation of background checks as required by the regulation entitled Background Checks for Licensed Child Day must be kept in each staff member's record. Staff #1's sworn disclosure completed three years ago was misplaced and missing from the staff's record.
Plan of Correction:	Will ensure to maintain sworn disclosure documentation in all staff records. Will find the missing document and place it in the file and the staff member completed another one during the inspection also.
