During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on September 9, 2025, and completed on September 9, 2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and four staff. The inspector reviewed five children?s records and two staff records on-site. This inspection document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-510-F Description: The medication authorization shall be available to staff during the entire time it is effective.
A prescription medication was observed for child #6. There was no written parent/guardian or physician authorization present for the medication. The medication had not been administered.Plan of Correction: Per the director: we have notified the parent to provide the authorization.
Standard #: 8VAC20-780-510-G Description: Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. One medication, an Epi-Pen was observed without a child's name, the dosage amount and the time or times to be given. The Center Director confirmed the medication belonged to child #5, currently enrolled in the program. Plan of Correction: Per the director: We will notify the parent today we need the documentation.
