Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 9/9/2025. The on-site inspection began at 10:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within one (1) business day from today, which will be the close of business on 9/11/2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	8VAC20-780-160-C
Description:	Staff members shall submit the results of follow-up tuberculosis (TB) screening or testing at least every two years. Staff #2's updated TB screening results were overdue by five months.
Plan of Correction:	Current process deters staff from missing TB during orientation. However, process has been included in staff returning from extended vacations to prevent future violations.
Standard #:	8VAC20-780-270-A
Description:	Areas outside the center shall be maintained in a clean and safe condition. A poisonous plant was growing along the exterior of the chain link fence bordering the Preschool playground. Several branches containing poisonous berries have extended through the fence into the children's play area near the slides and are accessible to children.
Plan of Correction:	Short Term: Hedges were trimmed to prevent berries from landing inside playground. Long Term: Work order has been placed and escalated to remove the entire tree from the property.
Standard #:	8VAC20-780-340-A
Description:	Staff shall always ensure children's care, protection, and guidance. On 9/9/2025 on the Preschool playground, a group of two year old children were observed playing near a poisonous plant with berries that had grown through the chain link fence. Staff #4 was supervising the children at that time and was unaware the plant and berries were poisonous. Management was aware of the plant's toxicity and submitted a work order on 9/5/2025 after concerns with children attempting to eat the berries. There were no actions taken to prevent children's access to the poisonous plant and berries or to inform staff of the poisonous plant.
Plan of Correction:	Short Term: Hedges were trimmed to prevent berries from landing inside playground. Additionally, portions of the playground will be blocked to deter children from navigating towards the berry tree. Long Term: Work order has been placed and escalated to remove the entire tree from the property.
