During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 9/8/2025 and completed on 9/11/2025 as part of the Probationary licensure period. The Department issued a Special Order on 1/17/2025 to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, and place the licensee on probation for a period of 6 months as a result of high-risk violations. The inspector reviewed 5 children?s records, 8 medical records and 5 staff records on-site on 9/8/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/18/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was a physical examination in the record of child #2 that was a year past the required timeframe.

Plan of Correction: PD will ensure that all physicals are in within 30 days

Standard #: 8VAC20-780-510-G
Description: Medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Child #1 had medication on site. There was no label on the medication.
Child #3 had medication on site. There was no label on the medication.

Plan of Correction: These have been corrected

Standard #: 8VAC20-780-510-G
Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.

Medication for Child 1 was onsite, but there was no medication authorization. The medication was administered during the expired period.

Medication for Child 3 was onsite, but there was no medication authorization. The medication was administered during the expired period.

Description: The center shall implement a monthly practice of evacuation drills.

The provider had not completed evacuation drills for months March 2025 through June 2025.

Plan of Correction: PD will ensure that all fire drills are completed each month.
