During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring/ inspection was conducted on August 28, 2025. The on-site inspection began at 3:20 p.m. and ended at 5:30 p.m. The inspector reviewed compliance in the areas listed above. There were 18 children present and 4 staff. The inspector reviewed 15 children?s records and 4 staff records on-site . This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 3, Child 10, and Child 11 enrolled for 4 months, did not have documentation of an immunization record on file. Plan of Correction: Children's records will be completed.
Standard #: 8VAC20-780-140-A Description: Repeat violation: Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 3, Child 5, Child 7, Child 9, and Child 10 did not have documentation of a physical. on file. Plan of Correction: Children's records will be completed.
Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. Child 8, Child 9, and Child 10, and Child 11 did not have documentation of the child?s proof of identity and age. Plan of Correction: Children's records will be completed.
Standard #: 8VAC20-780-90-A Description: A written agreement between the parent and the center shall be in each child's record by the
first day of the child's attendance. Child 1, Child 3, Child 5, Child 7, Child 8, Child 9, Child 10. Child 11 and Child 14 did not have a signed agreement between the parent and the center in each child's file by the first day of attendance that includes: 1. An authorization for emergency medical care should an emergency occur, 2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible, and 3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.Plan of Correction: Children's records will be completed.
