During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 09/04/2025 and completed on 09/12/2025. The on-site inspection began at 9:00 am and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 39 children present and 7 staff. The inspector reviewed 5 children?s records on-site and 7 staff records electronically on 09/12/2025. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-240-A Description: REPEAT VIOLATION
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff 4 has not yet completed this course which is overdue by 5 months, and Staff 6 and Staff 7 have not yet completed this course which is overdue by 1 month.Plan of Correction: Staff will obtain training.
Standard #: 8VAC20-780-245-L Description: There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily heath observation of children.
There was no staff member on duty who has obtained instruction in performing the daily health observation.Plan of Correction: Staff will obtain training.
Standard #: 8VAC20-780-330-B Description: Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
The resilient surfacing (mulch) under the playground climbing equipment measured less than one inch and ground was visible.Plan of Correction: Provide will increase mulch under equipment.
