Comments:

An unannounced, on-site monitoring inspection was initiated on September 11, 2025 and completed on September 11, 2025. The on-site inspection began at 11:30 am and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 15 staff. The inspector reviewed 7 children?s records and 4 staff records on-site on September 11, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Description:	Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for seven children who were in care in September 2025 were missing the following required components: documentation of viewing proof of the child's identity and age.

Description:	Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the first date of attendance. Seven children's records did not have first date of attendance documented.

Standard #:	8VAC20-780-240-C
Description:	Staff shall complete orientation training on required topics prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff #1 has been employed for 3 1/2 years and has not completed orientation training. Staff #2 has been employed for 8 months and has not completed orientation training. Staff #3 has been employed for 15 years and has not completed orientation training.
