During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Technical assistance provided for other areas of the physical plant to watch or address before worsening.

Comments:
An unannounced inspection was initiated on August 28, 2025 and completed on September 12, 2025 in response to a complaint received by the licensing office on August 26, 2025 relating to allegations of condition of the physical plant. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 19, 2025. A POC submitted after this date will not appear on the public website.

Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Children were observed playing on the playground during the inspection. Areas on the playground that were in an unsafe condition included:
-Cracking/splintering wood along the border of the sandbox.
-Bent fencing extending over the top railing of the fence.
-Rust along the bottom of the green metal railings on the equipment.
-Peeling/chipped green paint areas: monkey bars, climbing handles, and climbing ladder.
-Peeling paint and rusty pieces on the music board piece.
-Crack in blue slide.
-Front gate and fencing along the front of the playground is rusted.

Plan of Correction: All conditions listed in this violation
will be repaired and/or restored no later than
October, 19, 2025.
-The perimeter timbers of the sandbox will replaced.
-Bent fencing will be covered with a polycap
fence top protection system.
-All green metal on the play structure will be repainted.
-The music board has been dismantled and removed.
-The crack in the blue slide has been covered & taped
while we search for a replacement.
-The front gate and chain link fencing along the
front of the playground will be repainted.
