An unannounced, on-site renewal inspection was initiated on September 11, 2025 and completed on September 11, 2025. The on-site inspection began at 11:55am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/22/2025 . A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-770-70-A Description: Documentation of at least two references to character, reputation, and competency are to be checked prior to employment. Staff #2, who has been actively working at the center for at least a year, only had one completed reference verification on file. Plan of Correction: Response from the director:

Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #3, under two years old has been in care for 8 months did not have additional immunizations in their record. Plan of Correction: Response from the director:

Standard #: 8VAC20-780-140-A Description: REPEAT VIOLATION
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. The record for child #1 who has been enrolled for over two years did not contain documentation of a completed physical.Plan of Correction: Response from the director:

Standard #: 8VAC20-780-160-C Description: REPEAT VIOLATION
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. The tuberculosis screening in the record for staff #4, staff #5, staff #7 and staff #8 was expired for approximately one month.Plan of Correction: Response from the director:
Staff was updated in TB test on 9/12 and 9/13.
Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Children?s records shall include the name, home address, and home phone number of each parent who has custody; when applicable, work phone number and place of employment of each parent who has custody; Name and phone number of child's physician; the phone number of two designated people to call in an emergency if a parent cannot be reached. The record for child #1, child #2 and child #3 did not have all the required elements.Plan of Correction: Response from the director:
The application was updated and complete on 9/11/25 upon pick up of child.
