During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 8/18/25. The on-site inspection began at 12:55 pm and ended at 2:55 pm. The inspector reviewed compliance in the areas listed above. There were 55 children present and 9 staff. The inspector reviewed 6 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Out of state background checks must be sent off prior to employment.
An out of state background check for staff A had not been requested prior to employment, this background check was 3 weeks overdue.Plan of Correction: We will send off for this background check by 8/20/25.
Standard #: 8VAC20-780-260-A Description: The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
The most recent fire inspection report is dated 2/29/24.Plan of Correction: I will contact the Fire Department to schedule the inspection.
Standard #: 8VAC20-780-330-B Description: Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
The fall zones on the three playgrounds were not maintained at the required 6 feet. Composite play structures, portable slides were within 1 to 3 feet of each other and the fences.Plan of Correction: We will move the equipment to be in compliance with the 6 foot fall zone.
Standard #: 8VAC20-780-510-I Description: The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided that the center has obtained written authorization from a parent/guardian.
The authorization form used did not have the center's name listed as being given the authority to administer an emergency medication to child A.Plan of Correction: We will change the form and get the proper signatures.
