During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussed school agers sharing a classroom with preschoolers. School agers will use the multipurpose room.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on September 11, 2025 after the licensing office received a request to modify the center's age range. The on-site inspection began at 8:40 a.m. and ended at 10:00 a.m. The inspector reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 23, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-280-B
Description: Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Disinfectant wipes and disinfectant spray were in an unlocked cabinet accessible to children.

Plan of Correction: We put safe locking to all cabinet from outside.

Standard #: 8VAC20-780-520-C
Description: Diaper cream shall be inaccessible to children.

Diaper cream was in an unlocked drawer, accessible to children.

Plan of Correction: We already put safety lock right away.

Standard #: 8VAC20-780-530-A
Description: At least one staff in each classroom where children are present shall have current certification in cardiopulmonary resuscitation (CPR) and current certification in first aid.

Staff #1 was working alone in a classroom where children were present. Staff #1 does not have current certification in CPR and first aid.

Plan of Correction: Schedule CPR 1st aid until she receive put a teacher has CPR 1st aid with the teacher.
