During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 09/16/2025 and completed on 09/16/2025. The on-site inspection began at 10:32am and ended at 12:08pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and five staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
There was no physical examination in the record for Child #4, who had been enrolled for eight weeks.Plan of Correction: Will have parent have Dr complete by 9/19/25.
Standard #: 8VAC20-780-245-A Description: Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.
Staff #2 obtained 6.5 of the 16 required hours for 2024.Plan of Correction: Moving forward will make sure all employees will complete 16 hours
