During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on September 11, 2025. The on-site inspection began at 8:45am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were 23 children present and 8 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #3, employed for five years, did not have documentation of an out-of-state Child Abuse and Neglect search from one state. Staff #3 had move out of the state of Virginia approximately 12 months ago and request was submitted, but no follow up when the search was not received.

Plan of Correction: I submitted the request, and when I did not receive the search with the others that had been mailed at the same time, I emailed requesting information. I did not follow up after the first follow up. I will resubmit by 9/16/25.

Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who has been attending the center for three weeks, did not have an immunization record on file.

Plan of Correction: Notified parents today that we need immunization record by 9/18/25.

Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 updated screening was due 11 months ago and Staff #4 updated screening was due 3 months ago. The updated screenings have not yet been completed.

Plan of Correction: Staff will be schedule for TB screening immediately and will have results by 9/15/25.

Standard #: 8VAC20-780-80-A
Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. At approximately 8:45am on 09/11/25, there were six children present in classroom #1, but only five children were marked present on the attendance record.

Plan of Correction: We will retrain staff by 9/16/25.

Standard #: 8VAC20-780-340-D
Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Classroom #1 did not have a qualified lead teacher with a group of six preschool children, ages 12-28 months.

Plan of Correction: The Assistant Director will cover until the staff completes training to become a qualified lead teacher.

Standard #: 8VAC20-780-350-B-2
Description: The required ratio for children ages 16 to 24 month is one staff per five children. During the inspection, Staff #2 was observed leaving classroom #2, which had seven children with one other staff (the youngest child 16 months old) for two to four minutes on three separate occasions.

Plan of Correction: Staff was corrected immediately and will be retrained today, 9/11/25.

Standard #: 8VAC20-780-500-B
Description: The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #2 cleaned the diaper changing surface and sprayed it with a bleach water solution, but did not allow it to dry for a period of at least two minutes.

Plan of Correction: We will retrain all staff by 9/12/25.

Standard #: 8VAC20-780-510-I
Description: The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center administers drugs only to the child identified on the prescription label in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration.

Classroom #1 had a prescription cream, dated 2/5/25, with instructions to discard after 10 days, and parent authorization was accepted on 6/17/25.

Plan of Correction: Prescription cream will be given back to the parents today and retrain staff to ensure they read the prescription label. Cream was removed from the classroom during the inspection.

Standard #: 8VAC20-780-520-C
Description: If diaper ointment or cream is used, these products shall be inaccessible to children. Classroom #1 (ages 12-24 months) had multiple containers of diaper ointment and cream on the bottom shelf of the changing table that was accessible to children.

Plan of Correction: Staff were reminded that all creams must be inaccessible to children and will retrain them. Violations corrected during the inspection.

Standard #: 8VAC20-780-560-G
Description: When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner; and content.

Classroom #3 had several containers of food that was not dated, and nine bottles that were not dated.

Plan of Correction: Teachers were redirected to correct the labels and will be retrain by 9/12/25.
