An unannounced, on-site monitoring inspection was completed on 09/08/2025, as a part of the licensure period. The on-site inspection began at 12:30 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 31 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
There was no updated documentation of immunizations in the record for Child #3(under two years of age).Plan of Correction: Updated immunizations will be requested.
Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
There was a screw protruding from a board at the base of the chain link fence along the perimeter of the playground.
There was peeling paint in the two toddler and the four year old class rooms.
The molding around the bathroom door frame in the four year old room was rotted.Plan of Correction: Repairs will be made.
