During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Comments:
An unannounced, on-site monitoring inspection was initiated on 8/21/2025 and completed on 9/18/2025. The on-site inspection began at 1:40 pm and ended at 4:00 pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 2 staff. The inspector reviewed 5 children?s records and 2 (all) staff records on-site on 8/21/2025. This inspection included document review, tour of the facility, interviews, observations, and measurements

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
A gallon sized pump bottle of sunscreen was located on the open top shelf of a bookcase in the area where children were being cared for. The sunscreen was labeled "Keep out of reach of children".

Plan of Correction: The sunscreen was relocated to a secured location during the inspection.

Standard #: 8VAC20-780-320-B
Description: Each restroom area provided for children must be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children.
There were no disposable towels and no air dryer available in the center for children to dry their hands after washing them.

Plan of Correction: The program director will request paper towels to be ordered and will borrow a small supply from the other site today to be used until the order arrives.

Standard #: 8VAC20-780-420-E-3
Description: Staff are required to request, at least annually, parent confirmation that the required information in the child's record is up to date.
The records for child #1 (enrolled for 2 years), child #3, (enrolled for 4 years), child #4 (enrolled for 8 years) and Child #5 (enrolled for 9 years) did not contain any documentation of parent confirmation that the required information in the child's record is up to date since they enrolled.

Plan of Correction: All parents of enrolled children are asked to inform the center of updates each year and at any time during the school year as changes occur. However, there is not a written document to show this unless a change has actually occurred.
Parents will be asked to review and complete documentation for the new school year in September 2025 noting any changes to the information. A copy of this documentation will be placed in each child's record.

Standard #: 8VAC20-780-510-I
Description: Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian.
There was no signed parent / guardian or physician Authorization to Administer Medication Form for 5 prescription medications and 3 over the counter medications(Some duplicates) stored at the center for emergency use that were listed on a signed Asthma Action Plan and a signed Allergy Plan.

Plan of Correction: The medications on site were for a child whose last day of attendance is the day of the inspection. They will be sent home with the parent today.
There was a misunderstanding of the need for medication authorization forms when a current signed Allergy Plan and current Signed Asthma Plan were already on file. In the future, the medication authorizations forms will be completed and signed in addition to the care plans signed by the parent and physician.
