An unannounced monitoring inspection was completed on 09/04/2025 in response to a self-report received by the licensing office on 08/13/2025 relating to staffing and supervision. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administration and background checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/26/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1 Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment. S1, employed for 28 days did not have a completed sworn statement. Plan of Correction: All staff shall have all required documentation completed and signed prior to first day of employment.
Standard #: 8VAC20-780-240-B Description: Staff shall complete orientation training required by 8VAC20-780-240.C prior to working alone with children and no later than seven days of the date of assuming job responsibilities.
S1, employed for more than 7 days, did not have documentation of required orientation training.Plan of Correction: Staff will have all training orientation and documentation completed within first 7 days of employment.
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 08/12/2025, C1 was out of sight and supervision for approximately 3-5 minutes. The child was found in a classroom alone by another staff member standing on top of a table turning the lights in the classroom on and off.Plan of Correction: Staff will do name to face checks to account for children. Supervision from supervisor monitoring twice a day and coach in the moment for accuracy.
Standard #: 8VAC20-780-340-F Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On 08/12/2025, a 4-year-old child was out of sight and supervision for approximately 3-5 minutes. C1 was found alone in a classroom by a staff member, who was not his classroom teacher.Plan of Correction: Staff trained on the standards of monitoring children under the age of 10 should be within sight and sound of the child.
