During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on 09/16/2025 and completed on 09/22/2025. The on-site inspection began at 10:55 am and ended at 1:12 pm.. The inspector reviewed compliance in the areas listed above. There were 27 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site and electronically on 09/22/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 09/29/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-2 Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
1. The center did not forward the applicants fingerprints and personal descriptive information for staff #1 to the Departments designee, Office of Background Investigations.
2. The center instead obtained a (fingerprint-based) criminal history records check directly from the Virginia State Police for staff #1 prior to employment.
3. A letter with a finding of eligible was not obtained for staff #1 from the Office of Background Investigations as required for a satisfactory criminal record check. The Code of Virginia requires that an applicant's fingerprints and personal descriptive information obtained pursuant to subdivision 2 of 22.1-289.035 be forwarded by the Department or its designee through the Central Criminal Records Exchange to the Federal Bureau of Investigation for the purpose of obtaining national criminal history record information regarding such applicant. The Department or its designee then reports to the child day center whether the applicant is eligible to have responsibility for the safety and well-being of childrenPlan of Correction: The staff member redid finger print through field print as required.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
There was a low plastic table on the toddler playground with a broken and cracked edge that could entangle clothing or snag skin.Plan of Correction: The table was removed from the playground.
